  • · Bill deadhead. Empty miles to the pickup still burn fuel and driver hours — the per-mile rate above applies to loaded + deadhead, like most broker rate sheets.
  • · Price the level of service, not the person. Wheelchair and stretcher trips need lift time, securement, and often a second set of hands — the multiplier covers that.
  • · Watch the broker line. A 15–25% broker cut turns a decent gross rate into a thin one fast. If margin goes red, renegotiate or decline the trip.
  • · Get your real cost per mile. Use the fleet cost-per-mile calculator — guessing this number is how fleets undercharge.
